AWA Liability Waiver

ASSUMPTION OF RISK AND RELEASE FORM

As a participant with the Anacortes Waterfront Alliance (AWA), I am fully aware that the activities associated with AWA involve certain risks of physical injury or death. I hereby assume all risk of injury, damage and harm to myself arising from such activities. I also hereby individually and on behalf of my heirs, executors and assignees, release and hold harmless AWA, its officials, employees and agents and waive any right of recovery that I might have to bring a claim or a lawsuit against them for any personal injury, death or other consequences occurring to me arising out of my volunteer activities.

I agree that I will be held liable for any damage to AWA’s equipment and I will be charged the full cost of repair or replacement.
